At Strive Rehabilitation Partners, we understand that optimal care happens through teamwork. That’s why we collaborate with physical therapy, occupational therapy, speech therapy, physicians, and case management to improve outcomes and enhance patient experience within acute inpatient rehabilitation programs. Our focus is on helping patients and families gain the emotional clarity, confidence, and education needed to return home or transition to the most appropriate post-acute care setting. We work closely with the program medical director and deliver individualized psychological treatment plans that support recovery goals, reduce barriers to discharge, and improve long-term outcomes.
