Consumer Rights and Regulatory Information

Our practice is licensed and regulated by the Texas Behavioral Health Executive Council (BHEC). Texas law requires that we provide consumers with clear instructions on how to access their health care records, contact the regulatory agency, and file a complaint.

Requesting Your Health Care Records

You have the right to request access to your health care records. To request your records, please submit a written request to our office. Requests may require identity verification and completion of an authorization form, as required by Texas law.
